It is with pleasure that we deliver your 2018/19 Student Accident Insurance Plan Master Policy and supplies. The 2018/19 insurance plan is currently in effect (refer to Master Policy for specific information, including start date of the program for your district).

Health Special Risk, Inc. (HSR) has been the administrator for this program and we look forward to an exciting 2018/19 school year. The benefits provided under this year’s policy (unless otherwise agreed upon) and Mutual of Omaha Insurance Company remains the accident insurance carrier.  You will notice a change to the policy language and format, for the 2018/2019 term as we’re issuing coverage using Mutual of Omaha’s newly approved policy.

HSR will continue to process and pay all claims from our Carrollton (Dallas), Texas office.

Enclosed for your use are:

1. District Master Policy.  This official document provides you with specific policy provisions, limitations, and exclusions pertaining to the 2018/19 program.  Please review thoroughly and let us know immediately if there are any discrepancies, as this document will govern all aspects of your program, including claims adjudication.
2. Claim Form

We hope these supplies will aide you in providing easy and straight-forward enrollment for your students.

School officials, trainers and coaches can “look up” student enrollments in the same manner they did via the HSR FAST TRACK web site at https://www.hsri.com/K12FT.  Cassandra Talton, 866-345-2680 ext. 5660 at HSR is your “Fast Track” point of contact, if you need assistance.  Fast Track Users can submit claim forms electronically on behalf of your student athletes directly into the HSR’s adjudication systems to expedite these accidents when they occur.
